Standardization of psychometric diagnostics in neurological rehabilitation - A computer-assisted neuropsychological test-battery deu deposit-license The aim of the current work was to investigate the possibilities of a standardization in neuropsychological diagnostics.<br />Different surveys among psychodiagnosticians have shown, that there are already informal test-standards present for different psychological fields. These standards have been used to put tests together for a test-battery, which also takes theoretical and statistical basics into consideration and therefore means quality assurance for neuropsychological diagnostics.<br /><br />Neuropsychologists were asked for their testing habits to assess the actual testing standard. Furthermore, the relationship between testpsychological data and the sociomedical decision was examined to assure, that the test-battery includes all of the prognostic relevant test-parameters.<br />Both the literature and the own work confirm the existence of an informal standard in using tests and the relevance of neuropsychological tests in predicting return to work. Due to these results and in order to assess the psychological variables in a sophisticated, reasonable, and economic way, the following tests were included in the test battery: Tests for attention (TMT or ZBFT, d2, KRT), memory (ZGV, ZGR, VEG, IST-2000R MF), higher cognitive functions (LPS 3, LPS 5, LPS 10, IST-2000R RE, IST-2000R WÜ, WST, TvH), and emotional variables (FPI-R).<br />The test-battery was divided into two sessions each lasting ninety minutes. In case patients have a reduced cognitive loading capacity, it was possible to leave out the last test of each session without influencing the other test results. The testing was computer-assisted and was performed in groups of two to four patients. During ten months, 265 patients of the NRZ Magdeburg (mainly rehabilitation-phase D) were tested.<br /><br />Statistical evaluations have shown, that the test-battery is reliable enough. The examination of validity confirmed the expected factorial structure, the predictive value of the battery, and relationships between the results in the test-battery and localization of impairment, age, education, and sex.<br />Observations during testing have shown, that even patients, who had no further experience in using computers, managed the computer-assisted tests. In addition, the testing in groups was practicable in neurological patients, especially in patients with similar levels of achievement. However, it would be useful for the practicability in clinical daily routine, if the test-instructions would be all interactive and would enable an adaptive selection of test-difficulty for the complex tests KRT and TvH.<br /><br />In summary the results of the current work show, that it is possible to develop a test-battery in order to standardize neuropsychological diagnostics, which consists of accepted and well-known tests and is sophisticated, reasonable, and economic for testing neurological patients.<br />However, it was not the aim to reduce the variety of psychodiagnostic methods in neurological rehabilitation to this test-battery. The battery is seen as a screening to get an overall view of the patients deficits and possibilities to compensate before more sophisticated individual testing is done.
